Your event signup is step one. This is the separate signature step—not another signup.

Complete this release before participating in a Play Way activation. It includes participation, liability, photo/video/audio/media, and volunteer conduct terms. Every volunteer signs once for the event listed.

Everyone reviews the same participation, liability and media terms below. If the volunteer is under 18, the parent or legal guardian completes the guardian signature section AFTER those terms. Adults sign in the adult signature section.

Participation, Assumption of Risk + Release of Liability — ALL VOLUNTEERS

I voluntarily choose to participate as a volunteer and/or participant in activities presented, organized, sponsored, hosted, or facilitated by The Play Way and, when applicable, H.E.L.P. the 11th. Activities may include games, movement, interaction with other participants and members of the public, public-space activities, event setup or breakdown, travel within an event area, and other community engagement activities.

I understand that participation involves risks, both known and unknown, including slips, trips, falls, collisions, physical exertion, weather and environmental conditions, interaction with other people, traffic or public-space hazards, illness, bodily injury, death, and loss of or damage to personal property. I voluntarily assume all ordinary risks associated with my participation, whether specifically described here or not.

RELEASE OF LIABILITY: To the fullest extent permitted by California law, I release, waive, and discharge The Play Way; H.E.L.P. the 11th when applicable; their owners, directors, officers, employees, contractors, volunteers, agents, affiliates, successors and assigns; and participating sponsors, event partners, venues, property owners, and host organizations (collectively, the “Released Parties”) from claims, demands, causes of action, liabilities, damages, losses, costs, or expenses arising out of or related to my participation, including claims based on the ordinary negligence of a Released Party, except for claims that cannot legally be waived.

COVENANT NOT TO SUE: To the fullest extent permitted by law, I agree not to file or maintain a lawsuit or other claim against a Released Party for any claim that I have released in this agreement.

I understand that this release does not waive liability that California law does not permit to be waived, including liability for gross negligence, reckless misconduct, or intentional wrongdoing.

Photo, Video, Audio + Media Release — ALL VOLUNTEERS

I understand that photography, video, audio, interviews, livestreaming, and other recordings may occur during The Play Way events and activities. I grant The Play Way and, when applicable, H.E.L.P. the 11th permission to photograph, film, record, livestream, interview, and otherwise capture my image, likeness, voice, statements, biographical information that I voluntarily provide, and participation.

I grant a worldwide, perpetual, royalty-free right to use, reproduce, edit, crop, adapt, combine, publish, display, distribute, transmit, broadcast, exhibit, license, and otherwise use these materials, in whole or in part, in any media or format now known or later developed, for purposes related to The Play Way, H.E.L.P. the 11th, their programs, events, public education, community engagement, publicity, advertising, sponsorship, fundraising, websites, social media, presentations, press and news coverage, documentaries, television, film, streaming, and other PLAY-related storytelling.

I understand that recordings may be edited, captioned, combined with other material, shared with event partners, media outlets, sponsors, production companies, or distribution platforms for authorized uses, and may be seen worldwide. I waive any right to inspect or approve the finished materials or the specific context in which an authorized recording is used.

I understand that I will not receive compensation, royalties, residuals, approval rights, or ownership rights arising solely from the authorized use of these materials. I release the Released Parties from claims arising from the authorized capture and use of my image, likeness, voice, and statements, including claims for right of publicity or privacy, except to the extent a claim cannot legally be released.

I understand that once content has been publicly distributed or shared by third parties, copies may remain in circulation and may not be fully retractable.

Emergency Medical Care

If I become ill or injured and I am unable to make decisions for myself, I authorize event personnel to obtain or provide reasonable first aid and to contact emergency medical services when reasonably believed necessary. I understand that The Play Way does not provide health insurance for participants and that I am responsible for medical costs incurred on my behalf, except where otherwise required by law.

Personal Property

I am responsible for my own personal property during the event. To the fullest extent permitted by law, I release the Released Parties from claims for loss, theft, or damage to my personal property arising from my participation, including claims based on ordinary negligence, except where such claims cannot legally be waived.

This is the same release, not a second registration. Signing here does not automatically clear the minor to participate.

I certify that I am the parent or legal guardian of the minor participant named above and have legal authority to sign this agreement. I consent to the minor’s participation. To the fullest extent permitted by law, I agree on behalf of myself and the minor to the assumption of risk, release of liability, covenant not to sue, emergency medical authorization, personal property provision, and photo/video/audio/media release contained in this agreement. I understand that my signature authorizes the capture and use of the minor’s image, likeness, voice, statements, and participation as described above.

Final Acknowledgment

I acknowledge that I have had the opportunity to read this agreement carefully and ask questions before signing. I understand that this agreement affects important legal rights, including the right to sue for claims released above. I sign voluntarily and intend this agreement to be binding to the fullest extent permitted by law. If any provision is found unenforceable, the remaining provisions will remain in effect. This agreement is governed by the laws of the State of California.
